Accessibility Statement
Last updated: June 5, 2026
Countryside Estates Nursing (“Countryside Estates,” “we,” “us,” or “our”) is a 111-bed skilled nursing and rehabilitation facility located at 750 US-64 (Hwy 64 East), Warner, Oklahoma 74469. We are committed to ensuring that our website is accessible to the widest possible audience, including residents, prospective residents, family members, caregivers, and visitors who use assistive technologies or who have disabilities. Accessibility is part of our broader commitment to dignified, person-centered care, and we believe everyone deserves equal access to information about the services we provide.
Our Commitment to Accessibility
We strive to conform to the Web Content Accessibility Guidelines (WCAG) 2.1, Level AA, published by the World Wide Web Consortium (W3C). These guidelines explain how to make web content more accessible to people with a wide range of disabilities, including visual, auditory, motor, cognitive, and neurological conditions. Conforming to WCAG 2.1 Level AA helps make this website easier to perceive, operate, and understand for all users.
As a Medicare and Medicaid certified provider and an equal opportunity provider, we also recognize our obligations to provide effective communication and equal access to programs and services. Our accessibility efforts are intended to support those obligations and to reflect our values as a community that serves seniors, individuals in recovery, and people with complex medical needs.
Accessibility Features of This Website
We have designed and built this website with accessibility in mind. Features include, but are not limited to, the following:
- Semantic HTML structure. Pages use meaningful, standards-based markup — including landmarks, headings, lists, and labeled regions — so that screen readers and other assistive technologies can interpret content in a logical order.
- “Skip to content” link. A skip link is provided at the top of each page, allowing keyboard and screen-reader users to bypass repeated navigation and move directly to the main content.
- Full keyboard navigation. Interactive elements such as links, buttons, and menus can be reached and operated using a keyboard alone, with a visible focus indicator to show the current position on the page.
- Reduced-motion support. The website respects the operating-system “reduce motion” preference. When that setting is enabled, animations, transitions, and automatic movement are minimized or removed to support users who are sensitive to motion. We avoid content that flashes or blinks.
- Descriptive alternative text. Meaningful images include text alternatives (alt text) so that their purpose and content are available to people who cannot see them, while purely decorative images are marked so assistive technologies can ignore them.
- Sufficient color contrast. Text and essential interface elements are designed to meet the WCAG 2.1 AA contrast requirements, improving readability for users with low vision or color-vision deficiencies.
- Responsive, resizable layout. The site adapts to different screen sizes and supports browser text resizing and zoom without loss of content or functionality.
- Clear, consistent design. Navigation, links, and labels are written in plain language and presented consistently across pages to support users with cognitive or learning differences.
Contact Form and Protected Health Information (PHI)
Please do not submit any Protected Health Information (PHI) through this website. The contact form on this site is a non-transmitting demonstration feature. It does not send, store, or deliver messages to Countryside Estates, and it is not a means of communicating health information, medical details, or any sensitive personal data.
For admissions questions, care concerns, or any urgent matter, please call us directly at (918) 463-5143. We are open 24 hours a day, 7 days a week.
Ongoing Efforts
Accessibility is an ongoing effort rather than a one-time project. We regularly review and test our website, work to remediate issues that are identified, and aim to incorporate accessible practices into new content and features as they are developed. We also welcome feedback from the public, which helps us identify barriers we may have missed and continue to improve the experience for everyone.
While we work to conform to WCAG 2.1 Level AA across the site, some content may not yet fully meet every guideline. Where any portion of our website is not accessible to you, we will make reasonable efforts to provide the information you need through an alternative method, such as by phone.
Third-Party Content
Some elements of this website, such as embedded fonts or content provided by third parties, may originate from external sources whose accessibility we do not fully control. We make reasonable efforts to choose partners and resources that support accessibility, and we encourage you to contact us if you encounter a barrier involving any such content so that we can assist you and seek a remedy where possible.
Requesting Accommodations or Reporting a Problem
If you have difficulty accessing any part of this website, need information in an alternative format, or would like to request a reasonable accommodation, please contact us. We will work with you to provide the information, service, or assistance you are seeking. When you reach out, it is helpful — but not required — to describe the page or feature involved, the assistive technology or browser you are using, and the nature of the difficulty.
