This Privacy Policy explains how Countryside Estates Nursing ("Countryside Estates," "we," "us," or "our") handles information in connection with this website, located at countrysideestatesnursing.com (the "Site"). Countryside Estates Nursing is a 111-bed skilled nursing and rehabilitation facility located at 750 US-64 (Hwy 64 East), Warner, Oklahoma 74469.
This policy applies only to information practices related to the Site. It does not describe how we handle medical records, treatment information, or other protected health information arising from the care we provide to residents and patients. Those records are governed by separate notices and by applicable federal and state law. Please read this policy carefully.
Important — do not send Protected Health Information (PHI) through this Site. The contact form on this Site is a non-transmitting demonstration feature. It does not send, deliver, or store any message, and it must never be used to share medical conditions, treatment details, insurance numbers, Social Security numbers, or any other Protected Health Information. 1. Scope of this policy
This policy covers information practices associated with visiting and interacting with the public-facing pages of the Site. It is intended to be transparent about a simple truth: this Site is primarily an informational brochure. It is designed to describe our community and services and to encourage you to contact us by telephone. It is not a portal, an account-based system, or a method for transmitting messages or medical information to our staff.
Our services include 24/7 skilled nursing; physical, occupational, and speech therapy; respiratory, ventilator, and tracheostomy care through our Signature respiratory program; dialysis support; respite and short-term stays; memory and long-term care; daily living assistance; and dedicated care for U.S. military veterans. Inquiries about any of these services should be directed to us by phone rather than through the Site.
2. The demonstration contact form
The Site may display a contact or inquiry form that asks for information such as your name, email address, telephone number, and a free-text message. The form is provided as a non-transmitting demonstration only. It illustrates how a contact form might appear, but it does not actually do any of the following:
- It does not transmit your entries to Countryside Estates, to any email inbox, or to any server controlled by us.
- It does not save, log, or store your entries in any database, file, or back-end system.
- It does not generate any message, ticket, or notification that a staff member will see or act upon.

3. No Protected Health Information (PHI)
Do not use this Site, including the demonstration contact form, to share any Protected Health Information or other sensitive personal information. This includes, without limitation, descriptions of medical or behavioral conditions, diagnoses, medications, treatment or therapy details, ventilator or dialysis needs, insurance or Medicare/Medicaid identifiers, Social Security numbers, dates of birth, or financial account details.
The Site is not a secure channel for health information and is not intended to support HIPAA-covered communications. 4. Cookies & analytics
We do not set our own advertising, tracking, or analytics cookies on this Site, and we do not operate a first-party analytics platform that profiles your activity across pages. We do not sell or rent personal information, and we do not use the Site to build marketing profiles about visitors.
The only third-party services this Site relies on are Google Fonts and, where a map is displayed, Google Maps. These services are described in the next section. The Site is intended for adults seeking information about skilled nursing, rehabilitation, and related services.
