What it is

Rehab in a skilled nursing facility

Countryside Estates provides short-term rehabilitation as a skilled nursing facility (SNF). That is different from an inpatient rehabilitation hospital: therapy is matched to what each resident can do each day, alongside round-the-clock nursing care, which suits people who need time and medical support to recover.

  • Physical therapy for strength, balance and walking
  • Occupational therapy for dressing, bathing and other daily skills
  • Speech-language therapy for communication and safe swallowing
  • 24/7 skilled nursing, medication management and wound care
Who qualifies

Medicare and short-term rehab

Original Medicare Part A can cover a skilled nursing stay when you had a qualifying inpatient hospital stay of at least 3 days (observation time does not count) and need skilled nursing or therapy every day. It covers up to 100 days per benefit period: days 1–20 at $0 and days 21–100 with a daily coinsurance ($217 in 2026). Medicare Advantage plans have their own rules. Read more about payment.

The stay

What a rehab stay looks like

Admission starts with an assessment and a care plan built around the resident’s goals. Nursing and therapy teams work together and track progress, and families are kept informed. As discharge approaches, we plan the move home or to the next level of care.
